The AI Error Nobody Is Talking About
Most conversations about AI failures centre on hallucination. The model invents a citation that does not exist. It fabricates a product feature nobody specified. It generates a confident answer with no basis in reality. The industry has spent considerable energy diagnosing hallucination, building retrieval systems to reduce it, and explaining to business audiences why even the best models sometimes make things up.
This is a real problem. But it is not the only problem — and for many organisations, it is not the most dangerous one.
There is a second class of AI failure that is harder to detect, harder to explain, and in some respects more consequential. We call it Semantic Equivocation.
What Is Semantic Equivocation?
In 1948, Claude Shannon demonstrated mathematically that the gap between what a sender means and what a receiver understands is not just inevitable — it is measurable. He called this equivocation: the quantifiable difference between what was transmitted and what was received. Because it is measurable, Shannon argued, it is also governable.
Semantic Equivocation applies that principle to AI. It is the gap between the meaning an organisation intends and the meaning an AI system infers from data, metadata, and context.
In plain language: lost in translation.
The critical word is infers. The AI is not guessing. It is not fabricating. It is reading exactly what it was given — and what it was given was already semantically wrong before it arrived. The definition did not match the operational reality. The entity was structured correctly but meant something different in practice than it declared in the schema. The field was valid, complete, and present — but its meaning had drifted between when it was encoded and when the AI acted on it.
The structure may be valid and the records complete, yet the model can still act on a different interpretation than the one intended.
How Semantic Equivocation Differs From Hallucination
The distinction matters — not just for accuracy, but because the two failures have entirely different remedies.

Hallucination is an output problem. Semantic Equivocation is an input problem.
When an AI hallucinates, it invents. When it operates under Semantic Equivocation, it executes — faithfully, correctly, on semantically misaligned input. The model did exactly what it was designed to do. The failure happened before the model was ever involved.
This is why Semantic Equivocation is the harder problem. A hallucination sometimes reveals itself — the invented citation cannot be found, the fabricated feature does not exist. Semantic Equivocation leaves no such trace. The data is there. The structure is valid. The AI is confident. And it is wrong.
Why the Gap Is Invisible
Three conditions combine to make Semantic Equivocation invisible to standard validation:
1. Structural validity. The data passes every schema check. The @id references resolve. The entity types are correct. The required properties are present. Nothing in the validation layer signals a problem because, structurally, there is no problem.
2. The AI has no access to intent. An AI agent reads what was declared. It has no way to know that “active customer” in your knowledge graph was informally redefined two years ago, or that “available” means something different in your warehouse than it does in your structured data, or that a service description was written for one audience but is now being consumed by another. The gap between declared meaning and operational meaning is invisible to a system that can only read declarations.
3. The output is plausible. Hallucinated answers sometimes feel off — vague, overconfident, or unverifiable. Answers generated under Semantic Equivocation feel entirely reasonable, because they are based on real data, correctly structured, accurately retrieved. They are just based on the wrong meaning of that data.
The result is an AI that is consistently, plausibly wrong — and an organisation that has no easy way to detect it.
The SIDC Diagnostic
Semantic Equivocation does not exist in isolation. It sits within a broader framework of AI failure modes, which we call the SIDC diagnostic:
S — Signal (Data layer). Is the data accurate, complete, and consistent? Garbage in, garbage out — the most familiar form of AI failure, and the most discussed.
I — Interpretation (Semantic layer). Does the AI correctly understand the data and objectives? This is where Semantic Equivocation lives. Models optimise proxies, not intent. Meaning gets lost in translation.
D — Drift (Temporal layer). Has the world, context, or data changed since training or deployment? The ground truth moves — and static definitions fall behind. Note that semantic drift (version-level definitional change in ontologies over time) is a related but distinct concept from Semantic Equivocation, which can occur at a single point in time regardless of version history.
C — Context (Systems layer). Do all components, systems, and agents share and maintain shared context across the full stack? This covers multi-model pipelines, enterprise systems (ERP, CRM, BI), semantic interchange standards, and external AI discoverability. The absence of shared context amplifies every other failure mode.
Most AI reliability conversations focus on Signal. SIDC makes explicit that Interpretation and Context are equally consequential — and that Semantic Equivocation is specifically the I-layer failure that structured knowledge graph governance is designed to address.
Shannon’s Insight Applied to AI Governance
What Shannon established in 1948 is directly applicable here: because equivocation is measurable, it is not an inevitable condition. It can be identified, quantified, and reduced.
This reframes Semantic Equivocation from an abstract risk into an engineering and governance problem with a concrete solution. The question is not whether the gap exists — it does, in every organisation that has not deliberately aligned its declared semantics with its operational semantics. The question is whether the gap has been measured and managed.
The organisations that will have AI they can trust are not necessarily those with the best models. They are those with the most rigorously governed knowledge graphs — where declared meaning and operational meaning are the same thing, and where someone is responsible for keeping them aligned.
